Job Description:

The Nurse Case Manager, part of a multidisciplinary team, provides client-centered assessments, treatment, and support for individuals with severe mental illness. Responsibilities include conducting mental and physical health assessments, health promotion, and managing medications in collaboration with the Team Psychiatrist.

Compensation: $40 per hour + Mileage covered (0.61/km from client to client)

Start Date: ASAP

Duration: 3-Month + Strong possibility of extension

Tenure: Full-time

Shift: 9am-5pm

Responsibilities

  • Initiate and coordinate client services per established guidelines, including assessment, care planning, and implementation.
  • Apply nursing techniques in compliance with legal and ethical standards, and contribute to the development and maintenance of Reconnect services.
  • Develop and sustain effective relationships with hospitals and community service providers, and advocate for clients to ensure quality care.
  • Oversee up to 10 clients, providing counseling, crisis intervention, and support in areas such as daily living activities and medication management
  • Maintain accurate client records, monitor and report on care plans, and ensure timely completion of laboratory work.
  • Provide education and consultation to clients, families, and program teams, and participate in staff meetings, research, and community education.
  • Adhere to confidentiality, health, and safety guidelines, and fulfill additional duties as assigned.
Technical and Behavioral Competencies Required:

  • Graduate of an approved School of Nursing with current RN certification from the College of Nurses of Ontario.
  • Minimum 3 years' experience in psychiatric nursing, with community psychiatric nursing preferred.
  • Proficiency in the nursing process, including assessment, planning, implementation, and evaluation.
  • Strong skills in diplomacy, negotiation, conflict resolution, and people management.
  • Computer literacy in MS Word, Excel, and email; ability to learn new software.
  • Excellent teamwork, communication, and analytical skills.
  • Experience with substance abuse, dual disorders, or legal issues is a plus.
  • Ability to work independently and collaboratively, with sensitivity to diversity and multicultural issues.
  • Regular access to a dependable vehicle and a valid Ontario driver's license.

Pay: $38.00 per hour
