Task description and requirements

Position: Nurse Practitioner

Tenure: Part-Time, Contract

Location: Toronto, ON

Key Responsibilities:

  • Conduct comprehensive health assessments throughout the lifespan, including holistic health history, physical examination, and appropriate screening and diagnostic tests.
  • Design treatment plans addressing physical and mental health concerns within the scope of available services.
  • Assess and manage episodic and stable chronic conditions, consulting with physicians when necessary.
  • Perform telephone screenings for intake, referrals, health care advice, and support.
  • Provide supportive and lifestyle counseling, including preventative and brief supportive counseling.
  • Write prescriptions and order laboratory tests within RN(EC) scope of practice.
  • Monitor and administer medication, participate in medication audits, and contribute to medication policy.
  • Make home visits to clients as needed.
  • Refer clients to specialists, hospitals, or community resources as appropriate.
  • Participate in community projects and case conferences to coordinate client services.
  • Maintain standards of client care according to College of Nurses standards and organizational policies.
  • Provide back-up clinical functions for other nursing staff.
  • Offer care at outreach clinics as needed.
  • Disseminate health and program information through various materials.
  • Educate and counsel individuals/families on lifestyle and nutrition issues.
  • Conduct regular visits to review health status for specific programs.
  • Utilize a health promotion approach, addressing determinants of health.
  • Maintain accurate records for timely communication with other professionals.
Technical and Behavioral Competencies Required:

  • Nursing degree from a recognized university or equivalent.
  • Current registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario.
  • 3-5 years of experience in community settings or a mix of community and relevant hospital/public health nursing.
  • Experience in crisis prevention and intervention, and conflict mediation.
  • Strong verbal and written communication skills.
  • Self-directed and able to work independently and as part of a team.
  • Proficient in Microsoft Office and Client Information Systems.
  • Valid driver’s license and access to a personal vehicle required.
