Description et exigences de la tâche
- Initiate and coordinate client services per established guidelines, including assessment, care planning, and implementation.
- Apply nursing techniques in compliance with legal and ethical standards, and contribute to the development and maintenance of Reconnect services.
- Develop and sustain effective relationships with hospitals and community service providers, and advocate for clients to ensure quality care.
- Oversee up to 10 clients, providing counseling, crisis intervention, and support in areas such as daily living activities and medication management
- Maintain accurate client records, monitor and report on care plans, and ensure timely completion of laboratory work.
- Provide education and consultation to clients, families, and program teams, and participate in staff meetings, research, and community education.
- Adhere to confidentiality, health, and safety guidelines, and fulfill additional duties as assigned.
- Graduate of an approved School of Nursing with current RN certification from the College of Nurses of Ontario.
- Minimum 3 years' experience in psychiatric nursing, with community psychiatric nursing preferred.
- Proficiency in the nursing process, including assessment, planning, implementation, and evaluation.
- Strong skills in diplomacy, negotiation, conflict resolution, and people management.
- Computer literacy in MS Word, Excel, and email; ability to learn new software.
- Excellent teamwork, communication, and analytical skills.
- Experience with substance abuse, dual disorders, or legal issues is a plus.
- Ability to work independently and collaboratively, with sensitivity to diversity and multicultural issues.
- Regular access to a dependable vehicle and a valid Ontario driver's license.
