Position: Nurse Practitioner

Position Overview:
Our client, one of the leading organizations in the community health services industry, is in search of a sharp Nurse Practitioner.


What will you be doing?

Performs thorough evaluations for individuals which includes gathering a detailed health history, conducting physical examinations, and arranging appropriate screenings and tests.
Manages both occasional health issues and stable chronic conditions commonly encountered in primary care settings, consulting with primary care physicians when necessary.
Conducts telephone screenings as needed, offering intake services, referrals, healthcare guidance for acute or chronic conditions, anticipatory advice, and support for reception.
Delivers supportive and lifestyle counseling, covering preventative guidance, brief supportive sessions, or short-term crisis intervention.
Writes prescriptions and requests laboratory tests within the authorized scope of practice.
Performs home visits for clients residing within the service area as required.
Facilitates referrals to specialists, hospitals, or community resources as appropriate.
Engages in community initiatives, events, and case meetings as a representative, ensuring seamless service coordination for clients.
Adheres to established standards of client care.
Assumes backup clinical responsibilities for other nursing staff during their absence.
Offers care at outreach clinics as required.
Disseminates health and program information to clients through posters, pamphlets, and community materials.
Educates and advises individuals and families on various topics such as lifestyle choices and nutrition.
Utilizes a health promotion strategy, addressing factors influencing health outcomes.
Keeps thorough and precise records to facilitate effective communication with colleagues.
Supplies statistical data when necessary.
Contributes to the creation of clinical protocols and guidelines as needed.
Engages in the planning and assessment of clinical initiatives and assessments as requested.
May undertake clinical management duties including infection prevention, sterilization protocols, and upkeep of clinical tools and materials.
Ad-hoc responsibilities as required


Must Haves:

5+ years of experience in a similar role in a community setting or blend of community and relevant hospital or public health nursing.
Degree in nursing from an accredited university or equivalent education.
Registered as a Registered Nurse with the College of Nurses of Ontario.
Proficiency in crisis intervention and prevention techniques.
Demonstrated ability in conflict resolution.
Strong verbal and written communication abilities.
Capable of working independently while also collaborating effectively within a team, engaging both internal and external stakeholders.
Proficient in computer applications such as Microsoft Office, Client Information Systems, etc.
Fluency in a second language is a bonus.
Possession of a valid driver's license and access to a personal vehicle is essential.
Must be open to working flexible hours which includes weekends, evenings, and statutory holidays as required.
