Task description and requirements
- Conduct intake assessments and evaluate patient needs upon admission and throughout their hospital stay.
- Develop and implement individualized care plans with specific short- and long-term goals, and monitor and adjust these plans based on patient progress and needs.
- Collaborate closely with medical staff, including physicians, nurses, and specialists, as well as family members, caregivers, social workers, and external agencies to address and resolve patient care issues.
- Manage patient cases, including advocacy, coaching, risk assessments, crisis intervention, and referrals to necessary hospital services and external resources (e.g., financial aid, rehabilitation services, community support programs).
- Facilitate communication among patients, families, healthcare providers, social workers, and other relevant stakeholders to ensure coordinated and effective care.
- Maintain detailed and accurate patient records, case notes, and other documentation in accordance with hospital policies and procedures.
- Utilize clinical assessment tools and frameworks to create and review care plans tailored to patient needs and hospital objectives.
- College diploma or university degree in social services, nursing, healthcare administration, or a related field; BSW or MSW preferred.
- Minimum of three years of experience in case management, care coordination, or a related healthcare support role.
- In-depth understanding of patient care needs, including those related to age-specific health issues and hospital services.
- Experience working with patients experiencing mental and/or physical health challenges.
- Strong verbal and written communication skills, with the ability to effectively liaise between patients, families, and healthcare teams.
- Demonstrated skills in case management, patient advocacy, and referral coordination.
- Ability to work independently and as part of a multidisciplinary team, including internal hospital staff and external community resources.
- Proficiency in computer applications (e.g., Microsoft Office, hospital information systems, electronic health records).
- Fluency in a second language is highly desirable.
- Valid driver’s license and access to a personal vehicle may be required depending on the role.
Contract length: 3 months
