Position Title: Navigation Assessment Clinician
Department: Clinical - Outpatient
Supervisor: Manager, Mood & Anxiety and Trauma Programs
Job Description
The Navigation Assessment Clinician will conduct mental health screenings with clients and make recommendations for interventions based on these assessments. Case coordination and communication across an inter-professional team and with community partners is an integral component of the role.
Duties and Responsibilities
• Conduct mental health screenings with individuals referred for various mental health concerns, including but not limited to trauma, mood disorders, anxiety disorders, and substance use
• Make recommendations for interventions, such as individual counselling, community resources, structured outpatient treatment, or inpatient treatment
• Liaise with community partners and occupational health services verbally and/or in writing
• Responding to inquiries and scheduling screening appointments
• Case management and coordination
• Completes detailed documentation and charting appropriately. Prepares reports or correspondence in a timely manner
• Participate in regular meetings and consultation meetings of teams
• Engages in personal professional development, maintains appropriate professional associations, and provides current documentation to the Human Resources Department
• Work in a virtual setting, Monday to Friday
• Other duties as assigned
Qualifications
• MSW preferred, or equivalent experience
• Current registration with a regulated professional College (i.e.: college of SW)
• Minimum 3 to 5 years of clinical experience
• Experience with measurement-based assessment and care
• Professional experience with public safety personnel
• Knowledge of the DSM-V
