JOB TITLE: Aftercare Coordinator - Hybrid (3 month contract)
DEPARTMENT: EHN Outpatient Services
RESPONSIBLE TO: Aftercare Manager
JOB DESCRIPTION
The Aftercare Coordinator is responsible for the onboarding and co-ordination of aftercare across the Edgewood Health Network. The aftercare coordinator will work with the clinical team to set clients up with online or in-person aftercare and create aftercare plans for each client.
JOB FUNCTION AND DUTIES
▪ Monitor discharge dates for all EHN inpatient facilities and ensure all clients are set up for aftercare
▪ Meet with clients in the last few weeks of treatment for an individual aftercare appointment, registering clients in aftercare group, onboarding them to Wagon, creating an aftercare plan and ensuring paperwork is complete
▪ Document all client interactions in KIPU
▪ Support client care team in ensuring KIPU files are created and up to date for aftercare clients
▪ Stay up to date on the status of aftercare groups across the network, including times, days and  space.
▪ Work collaboratively with EHN inpatient and outpatient facilities
▪ Work with aftercare manager, operations director and inpatient facilities to problem solve  workflow and client issues.
▪ Other duties as assigned.
 Education
▪ Post-secondary education in administrative studies, business administration, Addictions 
  Counselor Diploma, Social Service Worker Diploma, or relevant experience
▪ Crisis Intervention Training
Skills
▪ Experience in addictions and mental health field
▪ Excellent customer services skills
▪ Experience with Electronic Medical Records systems
▪ Strong organizational and time-management skills
▪ Proficient in MS Office products (Excel, Word, MS Outlook, PowerPoint)
▪ Detail-oriented
▪ Motivational interviewing skills preferred
