Setting:

This position entails community-based work, involving 3-4 visits per day either at clients' homes or hospitals, depending on their location (Must have a car)

Area of Focus: Westend of Toronto (Bathurst St to the east, St Clair Avenue to the north, the Humber River to the west and Lake Ontario to the south)

Compensation: $40 per hour + Mileage covered (0.61/km from client to client)

Start Date: ASAP

Duration: 3-Month +possibility of extension

Tenure: Full-time

Shift: 9am-5pm

Job Description:

The Nurse Case Manager is a member of an interdependent, multidisciplinary team

serving clients with severe and persistent mental illness; provides client-centered

assessments, treatment, rehabilitation and support services based on Reconnect

program guidelines; is responsible for specialized functions including conducting mental

and physical health assessments; health promotion and providing medication

management and administration in conjunction with the Team Psychiatrist.

Responsibilities include, but are not limited to, the following:

  • initiates service to clients according to established guidelines;
  • treats clients by using correct nursing techniques that are in compliance with all
legislated requirements, the practice and ethical standards of the College of Nurses

of Ontario and specific Reconnect procedures and policies;

  • participates in the on-going development of Reconnect services in terms of the
medication room, supplies, medical records, resource files and policy and procedure

issues;

  • assists in the development and maintenance of effective linkages with hospitals and
community service providers;

  • assesses and evaluates client needs, and develops and revises client care plans
accordingly;

  • identifies and communicates significant abnormalities in the client data to the
appropriate team members;

  • ensures implementation and evaluation of case plans based on a team approach;
  • identifies and reports irregularities or mistakes in client care to the appropriate team
members, completes required documentation, and ensures appropriate follow-up;

  • monitors the accuracy and timely completion of laboratory work;
  • serves as a client advocate to ensure that quality care is provided to clients;
  • works in the community as the primary case manager for up to ten [10] clients;
  • provides program teams, clients, families and others with specialized consultation /
education related to nursing issues;

  • develops, implements and documents treatment plans in conjunction with the client
and other program team members

Job Type: Full-time

Pay: $38.00-$40.00 per hour

Expected hours: 40 per week
